5K - 4th grade Girls
 
Registration and Evaluation Nov. 4th & 5th, 2011
Friday between 5pm - 8:30pm
Saturday between 9am - 11am
 
Season Information:
First Week of Practice is Jan. 9th, 2012 
Last Game is March 17th, 2012
Award Celebration is March 19th, 2012
 
Early Registration Fee is $65 per player
 
Late Registration Fee is $75 per player
*Registration fee includes uniform and mock turtle neck.
 
Early Registratoin Instructions (before Nov. 5th, 2011):
  1. Click the registration form link and print.
  2. Complete the registration form.
  3. Attend either the Friday, Nov. 4th or the Saturday, Nov. 5th registration and evaluation night.
  4. Present both the completed registration form and the $65 player payment at the registration and evaluation night.
Late Registration Instructions (after Nov. 5th, 2011):
  1. Click the registration form link and print.
  2. Complete the registration form.
  3. Click the evaluation form link and print.
  4. Complete the evaluation form.
  5. Present the registration form, evaluation form, and $75 player payment to the church office Mon. through Friday between 9am - 3pm.
